REBUILDING DAY FORMS
The following forms are used on our project days. Most documents are provided in a portable document format (PDF) and require Adobe® Acrobat® Reader.

HOMEOWNER APPLICATION
Homeowners should complete this form to request Rebuilding Together Seattle services.

NON-PROFIT FACILITY APPLICATION
Non-profit organizations applying for facility repair and rehab assistance from RTS should complete this form.

VOLUNTEER OPPORTUNITIES FORM
If you or a group would like to volunteer with our organization, please complete one of our volunteer forms and mail, fax, or email it to our office. After we receive and process your form, we will be in touch! Thank you for your interest in volunteering for RTS!

RT NATIONAL SAFETY MANUAL
Rebuilding Together's first priority is ensuring the safety of our volunteers and clients. Please refer to the National Safety Manual for more information.

VOLUNTEER WAIVER FORM
All volunteers 18 and older must complete a volunteer waiver form prior to participating at a site (some waivers are filled out in the morning on site). Please contact your project's volunteer coordinator to see how forms are being collected for your project.

PARENTAL WAIVER/CONSENT FORM
To volunteer at a work site for RTS, volunteers must be 14 years of age or older. Volunteers ages 14-17 should fill out this form completely with their parents, and give it to the project volunteer coordinator (in advance) or bring it to the work site the day of.

Please note: RTS does not permit volunteers under the age of 14, infants, or pets to be on site on rebuild projects. Thank you for your cooperation.
